The medical team of CMED is formed by a multidisciplinary group of medical professionals related to the digestive system and with the diagnosis of their diseases.
Gastroenterologists, children's digestives, general and digestive surgeons, specialists in endoscopy and functional digestive tests, radiologists and psychologists. All work together to give a service and a personalized treatment to our patients.
History - Reason to be
The Medical-Surgical Center for Digestive Diseases arises from the combination of the experience of Dr. Guerra Flecha, the innovative ideas of his son, Dr Guerra Azcona. And the passion of both for medicine and surgery of the digestive system.
The need for a monographic space in this specialty has pushed the team of Dr. Guerra Flecha to create a revolutionary concept in the area of diseases of the digestive system, attracting other professionals of proven reputation to collaborate in the initiative.
Thus, this center dedicated exclusively to the study, diagnosis and treatment of the different diseases of the digestive system, combines the experience and professionalism of its specialists with the most modern exploratory techniques and diagnostic machinery.
At CMED we offer a medical service aimed at excellence in the treatment of digestive diseases. An integral concept that seeks to cover all the needs within the gastrointestinal field.

2000 - 2006. He worked for the General Surgery and Hepatic Transplantation Department at Hospital Curry Cabral in Lisbon, where he completed his speciality in Hepatobiliary Surgery and Hepatic Transplantation, by taking part in more than 250 transplants. During this period, he also brought Minimally Invasive Thyroid Surgery procedures to this centre, where he completed 80 medical interventions of this kind between 2004 and 2006. He also attended numerous conferences and was the author of several publications on General Surgery, focusing on the area of Hepatic Transplantation.
2006 - Present date. He joined Hospital Ruber Juan Bravo 39 (formerly, Hospital San Camilo), as Co-head of the Surgical Emergency Department, along with Dr. Guerra Flecha. Both introduced the concept of Digestive System Comprehensive Assistance, by combining the specialities of Gastroenterology (Dr. Guerra Flecha) and General and Laparoscopic Surgery (Dr. Guerra Flecha and Dr. Guerra Azcona).
2006 - Present date. He contributed as a surgeon to the Cancer Area at Fundación Tejerina (by implanting reservoirs for Chemotherapy).
2007 - Present date. He was appointed as member of the Hospital Infection Control Committee at Hospital Ruber Juan Bravo 39 (formerly, Hospital San Camilo).
2010. CMED, Centro Médico-Quirúrgico de Enfermedades Digestivas (Digestive Diseases Medical and Surgical Centre), is founded and Dr. Guerra Azcona introduces the concept of "High-resolution consultations" in the area of Gastroenterology and General Surgery, in order to streamline and optimise the care provided to patients of these specialities.

Hobbies
Dr. Guerra Azcona devotes his spare time to his wife and four children and to his hobbies, including cooking, skiing and golfing.
What is your opinion about the current state of this medical speciality? What about its future?
Currently, we have to move towards efficiency and highly specialised medical consultations, in order to provide patients with the highest efficiency in diagnosing, and with greater time and resource optimisation. The field of private medicine requires going "beyond" the traditional approach of one-doctor consultations. Coordinated multidisciplinary teams are necessary to "solve" patients conditions without wasting their time having to attend several centres in order to run all the required diagnostic tests.
In the particular context of the Digestive System, I feel integrating all those specialities involved in these types of diseases is essential to provide patients with a "comprehensive response".
What do you like the most about this career? What about CMED?
What I like the most about my career is that, since it is vocational, it brings fulfilment to my life, both professionally and personally.
Regarding CMED, as its Co-founder, I find the spirit and values behind its creation to be the aspects that appeal to me the most about the project. Also the firm commitment to offer patients the possibility to run their consultation, diagnosis and treatment with the same multidisciplinary team, so that all professionals involved can be well aware of the medical condition afflicting each patient. This is the reason why CMED is staffed by gastroenterologists, radiologists, experts in Traditional and Endoscopic Techniques of the Digestive System, as well as by a team of general surgeons.

What is your opinion about the current state of this medical speciality? What about its future?
The part of Medicine focused on the digestive system is a very broad speciality, which encompasses two great areas: Gastroenterology and Hepatology. Technological advancements over the last decades have driven the development of those techniques used within this speciality, namely, Endoscopy, Ultrasound and the combination of these two: Endoscopy Ultrasound.
Regarding Endoscopy, its progress can be divided in two stages: from "Endoscopy Diagnosis" to "Endoscopy Treatment", which allows us to treat bleeding lesions, remove polyps, place prosthetics to alleviate tumours and remove gallstones from the biliary duct, among other uses. This therapeutic side has offered quicker, simpler, less aggressive and more cost-efficient alternatives to surgery, which cause less complications and, consequently, can be used in a complementary way.
In the future, this speciality will move towards colon and liver cancer prevention Genetics will undoubtedly be integrated into doctors work, as well as into those concepts related to pharmacogenetics

We will have to deal with emerging conditions, such as morbid obesity. and the increased incidence of inflammatory bowel disease and the pathology of the ageing process.
What do you like the most about this career? What about CMED?
What I love the most about my career is the simple notion that my work is aimed at helping people heal or at least relieve their suffering, when the former is not possible.
The possibility of building a multidisciplinary working group, which fosters collaboration and chooses the option best suited to each patient and our own experience is the aspect I find the most interesting about this project.

What is your opinion about the current state of this medical speciality? What about its future?
Currently, Digestive Medicine stands at a critical juncture, since new technologies open new possibilities and we need to know how to harness them. I have described this moment as critical because we must be able to integrate the humane knowledge of doctors into this new approach. We shall be very mindful that there is a risk that this career becomes dehumanised and that the standardisation of the cause and effect of every result may be established, without assessing the circumstances in which these results were gathered.
In order to overcome this and other challenges, digestive medicine is moving towards the training of doctors that are highly specialised in the different areas of the digestive system: Hepathology, inflammatory bowel disease, Motor Dysfunction Units, pancreas, etc.

What appeals to me most about this career is the ongoing process of hypothesis formation that takes place during each medical diagnosis, as well as its subsequent analysis and resolution.
This is one of the reasons why this project is particularly interesting, since it fosters communication and consultation among its different specialists, always within the area of digestive medicine. Likewise, the quick execution of complimentary tests, with direct access to their results available to doctors, cuts time between the first consultation and the diagnosis and subsequent treatment, which results in a marked improvement of patients quality of life.

What is your opinion about the current state of this medical speciality? What about its future?
Radiology is a contrasting speciality of medicine On the one hand, some of its techniques and devices have drastically evolved, such as the scan while others have remained virtually unaltered (barium enema and conventional x-rays).
Radiology faces the challenge of combining this technological evolution and its profitability, since these advancements often require high cost devices, which are not integrated into the market as quickly as desired. In any case, this speciality is moving towards the intensive use of technology and preventive medicine.
What do you like the most about this career? What about CMED?
Being able to help patients by means of Diagnostic Imaging is what I love the most about my career. Each patient is unique, and consequently also different. Therefore, our daily work is never a routine one, every case poses a new challenge and every little detail obtained from diagnostic radiology adds up in order to reach a final accurate diagnosis.
Regarding this project, first I would like to highlight the great professionalism and humanity displayed by its team, as well as its agility in diagnosing and treating patients pathologies, thanks to the new technologies available at the centre. The size of the centre and the spirit behind its team fosters a friendly and humane assistance for every patient, thus resulting in a very comfortable atmosphere.

What is your opinion about the current state of this medical speciality? What about its future?
Endoscopy represented a milestone in Gastroenterology. It quickly became a simple, safe, accurate and affordable alternative to diagnose lesions of the gastrointestinal tract and made it possible to collect representative tissue samples for histological studies, facilitating the distinction between benign and malignant lesions, and early-stage or late-stage ones.
However, future Gastroenterology has yet more to offer. The use of genetics is already a reality for conditions, such as: celiac disease, lactose intolerance, inflammatory bowel disease and multiple polyposis syndromes. There will be further progress regarding the diagnosis and treatment of hepatitis B and C. Faecal microbiota transplants will be used beyond the treatment for those cases of pseudomembranous enterocolitis that are refractory to conventional treatment, and it may well become a therapeutic alternative for obesity, diabetes mellitus and inflammatory bowel disease. Recent advancements in neurophysiology of the digestive tract, the increased understanding of the role played by already discovered neuropeptides and by those yet to be uncovered, as well as the real prospects of pharmacologically altering their mediated responses, anticipate changes in the way obesity and functional disorders of the digestive tract are dealt with. We seem to be approaching the digestive pathology of the ageing process, and we, along with cardiologists specialising in haemodynamics and interventional radiologists, shall face the challenge of resolving gastrointestinal vascular disorders.
What do you like the most about this career? What about CMED?
I realised my call to medicine at an early age, during my elementary and secondary education. This vocation reflects that I was inclined to accept a specific way of life. The well-known idea of protecting others from getting ill, of curing their illnesses when the aforementioned circumstance is not possible, or of, at least, relieving their suffering when healing is beyond our reach and the sometimes ignored noble desire to be needed and fulfil those needs are among the reasons for this vocation.
When I decided to pursue my specialisation in Gastroenterology, I had already completed my residency in Internal Medicine and I had experience dealing with pathologies related to other specialities, such as: Neurology, Cardiology, Pulmonology, Rheumatology, Endocrinology and Nephrology. Out of all of them, I was particularly interested in digestive pathology, which I thought was a combination of reflective thinking and action, since it offered the training needed to perform examinations, such as endoscopy and ultrasound, which could be used as a way of instantly confirming or excluding a presumed diagnosis, or as a therapeutic modality to solve the conditions afflicting patients.
CMED is an innovative organisation that comprises a highly qualified group of experts in different areas, in order to study, diagnose and treat the vast, prevailing and very diverse digestive pathology. LMedicine is the most noble and comprehensive career there is. Directly working with people is the aspect I love most about it. In our everyday lives, as doctors, we seek to take part in the health/disease processes of our patients. We can help people, both by preventing and healing conditions.

What is your opinion about the current state of this medical speciality? What about its future?
My subspecialty is broad inasmuch as it encompasses the areas of Paediatric Gastroenterology, Hepatology and Nutrition. The progress of laboratory techniques and technological advancements have resulted in the increase of diagnosis of some conditions, such as celiac disease, and have made it easier to conduct some digestive tests on children, which previously were more complicated.
On the other hand, we are seeing an increased incidence of conditions related to food allergies. Finally, we surely cannot forget that my patients are mostly children, thus causing the least harm possible is essential..
What do you like the most about this career? What about CMED?
What I love the most about my career is helping my patients and their families feel better, dealing with their conditions, so that they can live their lives as normally as possible. Regarding CMED, the aspect I find the most appealing about the project is being able to work within a professional team focused on digestive conditions, which strives to provide patients with the best diagnosis and treatment possible.
The addition of a paediatric gastroenterologist to this team would expand the age range of those patients that will be able to benefit from CMEDs care services.

Digestive medicine has undergone many dramatic changes in recent years, both regarding diagnostic test and precision and new treatments available nowadays It is also the case that, currently, there is an increasing trend towards conducting more diagnostic tests to choose the most suitable treatment, given that new drugs already on the market deal with many conditions that previously required surgical treatment..
On the other hand, there are other conditions for which surgical repair was not previously indicated, such as gastroesophageal reflux disease, but that, however, nowadays are dealt with new accurate surgical techniques, which involve minimal morbidity and dramatically improve the general quality of life of patients.
Better understanding of epidemiology, risk factors and the natural evolution of digestive neoplasms makes it necessary to implement screening programmes among general and high-risk groups, which create more work for gastroenterologists.
In recent years, the trend in medical and nursing sciences has been towards the creation of multidisciplinary working groups, which regarding this speciality mainly comprise a gastroenterologist, a digestive surgeon, a radiologist and, in certain cases, an oncologist.

What is your opinion about the current state of this medical speciality? What about its future?
In my opinion, my speciality is experiencing rapid growth and development. Nutrition-related issues are increasingly more common and urgent than ever before (particularly those related to obesity and food intolerances), however, due to this situation, there is also growing awareness of the critical importance of healthy eating among people, who are now more likely to take this aspect into account and visit nutrition specialists in order to improve or treat numerous aspects of their health.
I believe my speciality moves towards higher specialisation (creating condition-specific and personalised nutrition plans: particular plans for each patient and ailment)..
What do you like the most about this career? What about CMED?
Working with people and fixing health-related issues are the aspects I enjoy the most about my career. I am really passionate about dealing and working with people.
Regarding CMED, the feature of this project that appeals to me the most is how seriously and professionally the centre is carried out, its state-of-the-art technology, the human warmth and integrity displayed by the personnel and the fostered convivial and relaxed atmosphere. What is your opinion about the current state of this medical speciality? What about its future?
Digestive medicine is undergoing constant change and evolution which enatils the development of a high adaptation capacity. There are currently diagnosis techniques which have stirred the scope of prevention, diagnosis and treatment of digestive pathologies. Multidisciplinary treatment is necessary to improve clinical attention and the quality of life of patients.
What do you like the most about this career? What about CMED?
What I like the most about my career is to know that thanks to our work we help people cure, sooth and modify their disease in a positive way. What attracts me the most about CMED is the possibility to work with a high quality interdisciplinary team with the aim of improving the diagnosis, treatment and handling of the digestive pathology and therefore help improve the quality of life of people.

What is your opinion about the current state of this medical speciality? What about its future?
Digestive specialty moves towards a greater interaction with other branches of medicine and from this approach we obtain initiatives such as the Program for the Reduction of Digestive Stress.
As science progresses, we understand better how different causes may create a same effect and the need to create medical teams combining professionals from different areas of expertise.

I find it absolutely rewarding to guide broken and damaged people through daily life and problems to help them learn how to use the resources they have available to care for themselves, cure themselves and mend themselves
When someone feels bad inside, at an emotional and state of mind level, thoughts are polluted, health is severely damaged and personal relations start to intoxicate. The person suffering from stress tends to feel overwhelmed by problems and unable to find a solution.
Most attendants to the Course for the Reduction of Stress feel a substantial change after the second class. They are not the only ones benefiting from it, also their environment: family and co-workers.
In CMED I have the opportunity to focus on people with digestive diseases who are indeed some of the most related to anxiety and stress and where we can achieve better results.

Anaesthesia has the challenge of providing comfort to the patient. The moment we achieve the milestone of the patient undergoing surgery with little impact on its daily life it will be thanks to the progress I hope this specialty, Anaesthesiology, will experiment. .

Working with patients daily is the aspect I enjoy the most about my career: being able to help them thanks to my knowledge, "research" activities associated with diagnostic processes, and the creative process that brings together surgery science and artistry.
The professionality and good work are the two main requirements for me and CMED fulfilling them makes me feel proud of being part of its Medical Team. On the other hand, when covering the whole range of digestive interventions, from the simplest such as endoscopy to the most complex surgeries, CMED offers me the opportunity to continue mastering my specialty.

What is your opinion about the current state of this medical speciality? What about its future?
The evolution of the speciality in these last 30 years has been amazing. We have changed from a medicine with little diagnosis resources, manual and very reactive to the incorporation of technologies and processes which allow preventive medicine.
In the next years I believe that medical teams will be ever more specialized and will progressively incorporate professionals of other disciplines (in our case: radiologists, psychologists, etc.) .
What do you like the most about this career? What about CMED?
What I like the most of my profession is to help people with something as vital as their health. I do not think that there are many other works giving higher personal satisfaction than seeing the happiness of a family after curing any of its members. This offsets by far the heartaches we also have to see.
In respect of CMED, continue working with Dr Guerra Flecha and being part of this new project gives me renewed energies and thrills. CMED is a new proposal in Spain and I believe that its technical, professional and human quality are enough motivation to any health professional interested in working here..
